Request Forgotten Password for EDC Web Form

To reset your password you will need your web form ID from the letter postmarked on May 1, 2022. There is a separate web form ID for each EDC form.

Please contact the EDC Program if you:

  • Did not receive a letter with your web form ID,
  • Do not receive an email within five minutes of submitting this form, as some email providers such as aol.com and charter.net block our system emails.

Send me a link to reset my password

We will send a link to the email address you provided when you first created your password. You will have 15 minutes to follow the link and choose a new password.
